The Breast Health Clinical Navigator (BHCN) is here to decrease anxiety and fear associated with the cancer journey and facilitate smooth transitions throughout care. We guide breast cancer patients with a suspicious finding, such as cancer, through the healthcare system to ensure timely diagnosis, treatment and survivorship care.
Our navigator brings years of oncology nursing experience to Shaw Cancer Center. Our goal is to facilitate an appointment within one week of a suspicious finding or the following Tuesday after a finding, during which time she gathers pertinent clinical data (such as scans and pathology) for review by Shaw's multi-disciplinary team including, but not limited to: breast surgeons, medical oncologists and radiation oncologists. We then coordinate appropriate appointments for the weekly Tuesday Breast Clinic. After appointments, we provide education on the designated treatment plan, coordinate follow-up appointments and insures the patient has appropriate support resources for moving forward, including emotional support, nutritional guidance and assessing challenges patients may face such as financial concerns, transportation and concerns with body image.
Upon completion of treatment, we provide the patient with a survivorship care plan that outlines cancer history and treatment as well as anticipatory guidance for future preventative healthcare needs. Our navigator acts as a single point of contact throughout a patient's cancer care and maintains an ongoing dialogue with her patients to facilitate improved communication amongst their providers in a timely manner.
