The form may also be faxed to (970) 470-6403. Please specify if you would like to pick up the copies in person, send them to another facility, or have us mail them to you.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Remember &#8211; you can access your medical information through the&nbsp;<a href=\"https:\/\/www.vailhealth.org\/patients-and-visitors\/patient-portal\" target=\"_blank\" rel=\"noreferrer noopener\">Vail Health patient portal<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Organizations<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">To initiate a request for medical records from Shaw Cancer Center, please fax your request on office letterhead with the patient\u2019s name, date of birth, dates of service, and types of records you would need. If requesting for multiple patients, please send separate requests.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Note:<\/strong>&nbsp;Requests for medical records are processed in the order they are received.
